There are some sims, and many townies who have the same name, both first and second. Despite looking different they have somewhat similarities. They could be Adbuct-ees who have been cloned, given different faces and sent into different places. (Another neighbourhood, a university, a vacation destination, or any other sub-neighborhoud.) Well, it depends somewhat. The Malcolm Landgraabs are all relatives for example. Other sims in TS2 have "hidden" copies of themselves. Could be "clones" could be not. Service sims (or at least maids) are actually based on long-dead people but "changed" to fit a new destiny. The Grady Elfman in SV was "based" on the Grady in RV, same for Kaylinn in TS2 was based on the one in TS3. Nobody knows why this happens, some NPCs use "genes" from one soul and some from another, for example. Hence why there are some NPCs with Goth as a surname. Their NPC DNA is actually part-Goth. You know how the Door of Life and Death makes a puddingface with traits, favorites, and same name as the sim that died? Also the Tombstone of Life and Death, which seem to be the same technology as the Door. Well the same Magitech actually. the Tombstone can spawn townies (and plausibly spawned the Pleasantwiew Townies and Service Sims among others) but can also make you pregnant and there's the option "Make Me Family CHild" it makes you part of a family - in Pleasantwiew it makes you into Bob Newbie's sibling i don't know for other towns. And i guess the various NPCs and Townies seem made with that technology too or well it would be Magitech instead. So i guess two NPCs who share a name are distantly related through their soul's DNA. The Tricou Teens? Jennicor has "of Life and Death" powers, and split Jon Smith's soul into parts that became these Teens. And for Kale Parrot (a NPC firefighter in Appaloosa who is the brother of a playable) well he does not have the hidden trait for Firefighters but Ashleigh Ashford does (just pyromaniac not immune to fire) so i guess Ashleigh is the one who was pieced down from the long-dead's souls - Kale prolly just ran away and decided to become a NPC-type firefighter even without the hidden trait, there was really no original AP male firefighter.
